Event description

Employee partially amputates thumb while using table saw

Investigation abstract

At 12:15 p.m. on August 4, 2021, in the warehouse, Employee #1, a supervisor, was using a Powermatic Table Saw to cut a small piece of plywood. During the operation, Employee #1's left thumb came too close to the blade, resulting in a partial amputation. Although the table saw was properly guarded, the employee was not using the provided push stick for cutting smaller pieces. The employee was treated without hospitalization.
