Event description

Employee is killed after struck by crane fixture that failed

Investigation abstract

At 5:30 a.m. on August 12, 2021, an employee had been operating an overhead bridge crane to install an injection mold in a press. Once the employee had installed the injection mold he was utilizing a crane to lift a fixture used to install the mold when the fixture failed. The employee was killed after being struck in the head by and pinned under the fixture.
