Employee with COVID-19 infection dies
Investigation abstract
Employee # 1 was working as a delivery driver. He worked during the two weeks prior to the onset of his symptoms on July 28, 2021. Employee # 1 contracted COVID-19, confirmed with test results on August 3, 2021. He was hospitalized on August 9, 2020, and then died at 2:30 p.m. on August 12, 2021. His employer reported the hospitalization and fatality to the state OSHA on August 12, 2021.
