Investigation abstract
At 6:40 a.m. on May 23, 2021, Employee #1 was working on the rig floor with a casing crew that had just finished a joint while running casing. The crew heard a noise and looked up to see the casing running tool (CRT) falling from the top drive. As the employees attempted to move out of the way, the CRT struck the stump on the rig floor and then bounced into Employee #1. The employee sustained a puncture injury to the back and an amputation of four toes on the right foot. Employee #1 was hospitalized for treatment.
