Employee suffers partial amputation of left arm while runnin
Investigation abstract
At 3:30 p.m. on July 6, 2021, Employee #1 was running a 1/4 inch strand wire across a highway for utilities installation when a northbound vehicle ran over the strand. The vehicle pulled on the cable, causing a partial amputation of the employee's left arm about 5 inches below the elbow. A required Traffic Control Plan was neither obtained nor implemented, and guidance from Section VI of the Manual on Uniform Traffic Control Devices was not followed.
