Investigation abstract

At 12:00 p.m. on May 27, 2021, Employee #1 was working at an agricultural equipment manufacturing facility. He was operating a JM Montgomery Roller (Model 7210H) to roll a 12-inch-wide by 116-inch-long, 16-gauge steel sheet into a circular shape for use on an agricultural duster. During the operation, his left hand was caught in the in-running side of the roller and was crushed. The control placement on the equipment allowed the employee's hands to come into contact with the rollers. He was hospitalized for treatment.
