Employee partially amputates finger during machine cleaning
Investigation abstract
At 10:30 p.m. on August 13, 2021, Employee #1 was cleaning a chain mechanism. The machine had not been properly deenergized, and when the employee's shoulder crossed a sensor, it activated the machine, causing the chain to move. This resulted in a partial amputation of Employee #1's right middle finger. The employee was hospitalized for treatment of the injury.
