Investigation abstract

At 4:30 p.m. on July 29, 2021, an employee working as a sales driver for a refrigeration equipment supplier was removing the back gate of a flat bed truck fitted with a removeable fence. The employee fell 4 feet from the loading dock to the ground. The employee later collapsed in the parking lot when leaving for the day. The employee was hospitalized to treat a head injury.
