Investigation abstract

At approximately 4:07 p.m. on June 30, 2021, Employee #1 was replacing and cleaning the De-Moulder when their right index finger was caught between the tray and a steel bracket. The incident resulted in the amputation of the employee's right index finger. The employee was hospitalized for treatment.
