Employee is killed when crushed in skid-steer
Investigation abstract
At 3:30 p.m. on August 17, 2021, an employee was standing on the bucket attachment of a skid-steer tractor facing the operator compartment and instructing a coworker on how to operate the skid-steer. The employee stepped on the left floor pedal of the tractor to demonstrate how raise the bucket. The bucket and lifting arms raised up quickly while the employee was still standing on the bucket. The employee's torso was caught between a cross-member on the lifting arm and the top of the tractor cab. The employee suffered crushing injuries to his chest and neck and died.
