Event description
Employee incurs multiple injuries when crushed by skid loade
Investigation abstract
At 7:00 a.m. on June 14, 2021, an employee was operating a New Holland skid loader when the throttle linkage slipped off. The employee accessed the engine compartment with the lifting arms raised to reattach the throttle cable. While in the engine compartment, the employee communicated to the foreman to throttle down; however, the foreman mistakenly thought the employee instructed to lower the arms. The foreman lowered the lifting arms, crushing the employee between the engine compartment and the arms. The employee sustained internal injuries to the liver, heart, and lungs, as well as multiple broken bones. The employee died just past 30 days following the accident due to the injuries sustained.
