Employee driving PIV fractures leg when collides with parke

Investigation abstract

At 12:50 p.m. on May 4, 2021, an employee working as a shipping and receiving clerk for a warehouse distribution center was driving an Electric Pallet Jack (EPJ) powered industrial vehicle (PIV) to move boxes on a pallet. The employee was a full-time, permanent employee of the employer. The employee was driving the PIV near the junction of two drive ways. While making a right turn, she crashed into a parked forklift. The employee was hospitalized for one week to treat a fractured left leg.
