Employee amputates fingers when caught in machine

Investigation abstract

At approximately 2:30 p.m. on August 23, 2021, an employee was attempting to clear a jam in the machine. The employee reached below guarding to clear a jam and her hand was caught in the sealer. The employee amputated her right-hand index finger between the tip and first knuckle and middle finger between the first and second knuckle. The employee was hospitalized for these injuries.
