Employee amputates two fingers during batch mixer cleaning

Investigation abstract

At 12:30 a.m. on August 13, 2021, Employee #1 was cleaning a batch mixer when the pneumatic discharge gate closed unexpectedly. This resulted in the amputation of two fingers as they were caught between the pneumatically operated discharge door and the steel lip of the batch mixer drum. The employee was hospitalized for treatment of the injuries.
