Event description

Employee is killed in explosion and fire

Investigation abstract

At 12:00 noon on August 25, 2021, an employee was welding a cracked weld on a dump truck when an explosion occurred and ignited his clothing. The dump truck owner saw employee was on fire, pulled him out from under the truck, drug him approximately 38 feet, and used a fire extinguisher to put him out. The employee was soaked in hydraulic fluid and diesel fuel. The employee died from his injuries.
