Employee with COVID-19 infection dies
Investigation abstract
At 8:00 a.m. on August 12, 2021, an employee who worked as a Forklift Operator effective COVID-19 Prevention Program in place. for a Logistics Warehousing Facility was found in his home dead due to COVID-19. The facility is a warehouse that primarily stores cases of bottled water for distribution customers. On August 2, 2021, the employee reported to work but within a few hours into his shift the employee began to experience symptoms. The employee was immediately sent home. On August 3, 2021, the employee took a COVID-19 test. On August 4, 2021, the employee received a positive test result. The employer conducted COVID-19 tracing and discovered one other COVID-19 related hospitalization of a temporary employee. The employer did not have an
