Employee is struck by hot metal carrier while chipping slag
Investigation abstract
At 6:00 p.m. on July 17, 2021, Employee #1, a 23-year-old refractory worker, was chipping slag out of Auto-pour Receiver #2 using a pneumatic chisel bit. The employee was working without a spotter to monitor overhead Hot Metal Carrier (HMC) traffic, and the anti-collision system that deenergizes the HMC track did not activate. While performing this daily task, the employee was struck on the left side of his body by the HMC, pinching him between the ladle on the front of the HMC and the chisel bit. The employee suffered a lacerated liver and was hospitalized.
