Investigation abstract
Three employees were working for a municipal government. Employee #1 worked as a employer learned of his death at 8:00 a.m. that same day. The employer informed DOSH of the fatality at 9:00 a.m. that same day. DOSH learned of the hospitalization on at 3:09 p.m. on that day, outside of the required reporting requirements. DOSH opened an investigation at 3:09 p.m. on September 10. During the opening conference DOSH was made aware of two other employees who had also tested positive for COVID-19. For all three employees, their last day worked was August 20, 2021. They called out from their next scheduled workday, August 23, 2021. Employee #2 noticed symptoms on August 21, and he was tested at a hospital on August 26. The test results came back positive for COVID-19, and he was admitted to the hospital at 12:18 p.m. that same day. He was discharged at 2:57 temporary maintenance worker. Employee #2 worked as a lead maintenance worker. p.m. on August 29. The employer was notified of the hospitalization at 4:56 p.m. on August 26. DOSH learned of the hospitalization at 3:09 p.m. on September 10, outside of the required reporting requirements. Employee #2 returned to work on September 27. Employee #3 noticed symptoms on August 20, and he was tested on August 25. He received positive test results that same day, but he was not hospitalized. He returned to work on September 15, 2021. Employee #3 worked as an equipment operator for the town. Employee #1's last day worked was August 20, 2021. He called out from his next scheduled workday, August 23, 2021. He had experienced an onset of COVID-19-related symptoms on August 19, 2021. He was tested on August 25, 2021, at a hospital. The test results came back positive for COVID-19. At 11:47 a.m. that same day, he was admitted to the hospital. His employer was notified at approximately 10:55 a.m. on the following day. Employee #1 died at 2:10 a.m. on September 10, 2021. His
