Investigation abstract

At 2:30 p.m. on May 5, 2021, Employee #1, assisting with the unloading of a 1,300-pound crated HVAC unit, was hand signaling to guide the forklift operator. The forklift operator failed to position the forklift forks deep enough into the load, causing it to become unbalanced as it was moved off the trailer. As the load began to tip, the employee rushed to the left side of the load in an attempt to balance it with his hands. However, the load ultimately fell on the employee, causing fractures to his left arm and left leg. Emergency services were called, and the employee was transported to the hospital for admission and treatment.
