Event description
Two Employees with COVID-19 infection die
Investigation abstract
On August 18, 2021, two employees who had been working in a warehouse reportedly COVID-19 outbreak was occurring and Employee #2 was the second positive case in that work group. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative. died from COVID-19 infection. Employee #1 had been working as normal and had worked a full shift prior to her death. Employee #1 last worked on July 30, 2021 and after developing symptoms had tested positive on August 2, 2021. Employee #1 notified her employer of the positive result on August 2, 2021. On August 5, 2021, Employee #1 was hospitalized and on August 18, 2021, she died. On September 20, 2021, Employee #2 who had been exposed to his COVID-19 positive girlfriend tested positive and notified his employer.
