Event description

Employee with COVID-19 infection dies

Investigation abstract

An employee was working as a nursing aide at a nursing care or skilled nursing facility. On September 7, 2021, he tested positive for COVID-19. He entered quarantine. On September 16, his employer learned from the employee's wife that the employee had been hospitalized. The employer then learned from one of the employee's coworkers that the employee had died. At 11:32 p.m. on September 16, an administrator of the facility reported the death to the state occupational safety and health agency. She did not know at what time the employee had died. She was advised that an inspection was open. The death was reported to the hotline at 11:23 p.m. that same day.
