Event description

Employee incurs multiple injuries when struck by falling car

Investigation abstract

At 1:20 p.m. on June 25, 2021, Employee #1 was performing a routine task of picking ground stock in aisle B of a warehouse. Aisle B had not been cordoned off while stocking was occurring in adjacent aisle A. While a forklift was in use in aisle A, a stack of flattened cardboard material came loose from a wooden pallet and fell into aisle B, striking the employee. Employee #1 sustained bruises and swelling to the head, neck, and upper arms. The employee was treated without hospitalization.
