Event description

Air handling unit falls over, kills one employee and causes

Investigation abstract

At 2:30 p.m. on September 22, 2021, two employees work to guide an air handling unit onto skates on the top of a building. The front side of the air handling unit was positioned on the skates and the employees were working to adjust the placement of the rear section on the skates. A crane operator was instructed to raise the air handler approximately one inch. As this was accomplished, a wind gust started and caused the air handling unit to tip over. Employee #1 was directly under the air handling unit when it fell over and was crushed and killed. Employee #2 received crush injuries to the head and body and was hospitalized to treat the injuries.
