Employee amputates leg after falling into vehicle track hole
Investigation abstract
At approximately 12:00 p.m. on June 30, 2021, Employee #1 was drying vehicles at the exit portion of a car wash when he lost situational awareness and fell into an open hole in the vehicle track. The hole was where the rollers cycle back underneath to the entrance of the car wash. Employee #1 was hospitalized to treat a leg amputatation.
