Employee sustains finger amputation while removing label fro
Investigation abstract
At 11:00 a.m. on July 16, 2021, Employee #1 was removing a label from the glue knife assembly and glue roller. The employee's left hand, which was holding the assembly open, was pulled into the moving machinery behind the glue roller. The employee sustained an amputation of the left hand middle finger at or above the first knuckle and was hospitalized.
