Investigation abstract
An employee was a caregiver at this nursing facility. The employee's last day of work was on August 22, 2021, the same day the employee tested positive for COVID-19. The employee was admitted to the hospital due to COVID 19 complications on September 11, 2021. The employee died from COVID-19 complications at approximately 9:00 a.m. on September 27, 2021.
