Event description

Employee sustains partial fingertip amputation while trouble

Investigation abstract

At 4:30 p.m. on July 14, 2021, Employee #1 was troubleshooting an issue with the Combi Box Maker on Line 17 in Room D. While the machine was running, the employee attempted to adjust a misaligned index gate. During the adjustment, a paddle on the conveyor came around and pinched his finger between the paddle and the back of the index gate, resulting in a partial amputation of the fingertip. Emergency services transported Employee #1 to the hospital, where he was admitted and treated.
