Event description

Employee?s finger is amputated while loading wire coil

Investigation abstract

At 3:55 p.m. on June 3, 2021, Employee #1, a shaver operator, was loading a new wire coil onto the drum of shaver #2 at a raw materials supplier serving the fastener and steel industries. After loading, the employee held the end of the wire to feed it into the machine and prevent twisting. While doing so, his finger was caught between the wire and the drum, causing an amputation of the left index finger. No witnesses observed the incident. The employee was hospitalized for treatment.
