Employee injures two fingers during tarp mechanism operation
Investigation abstract
At 11:00 a.m. on August 12, 2021, Employee #1, employed by the City of Berkeley's Zero Waste Division, was involved in an incident while performing commercial refuse services. After loading a roll-off box filled with metal material onto unit #6360, equipped with a Donovan Hy Tower SL T-Bar, Employee #1 manually rolled a tarp over the box. While attempting to lower the T-bar using the truck's controls, which were unresponsive, Employee #1 climbed onto the truck's frame to investigate the issue. As he positioned his left hand on the bar, it unexpectedly descended, crushing his left middle and index fingers and causing fractures. The employee was treated without hospitalization.
