Event description

Employee falls through floor hole and is killed

Investigation abstract

At 5:30 p.m. on September 17, 2021, an employee worked to frame a wooden gable for a dwelling. A piece of plywood was cut from the gable window opening that was then laid on the floor. The employee attempted to kick the cut section of plywood free and fell through the hole and approximately twenty-one feet down an elevator shaft. The employee received serious internal injuries and was killed.
