Employee with COVID-19 infection dies
Investigation abstract
An employee was working for a child daycare service that was operated by a reported her death to DOSH over the telephone. The employer reported the death within the required time. That same day, at approximately 2:15 p.m., DOSH arrived to investigate. The employer had implemented mitigating measures for COVID-19 as required. The investigation did not involve an accident, as it was illness related. No accident-related citations were to be issued, but other citations were to be issued. government agency. She worked as an associate teacher at a small pre-kindergarten school, with 15 employees onsite. The workers there provided classroom instruction. The employee worked directly for her employer. One of her coworkers began to exhibit symptoms related to COVID-19. On September 2, 2021, when the employee was identified as a possible close contact, the employee, who was unvaccinated, was sent home to quarantine.
