Investigation abstract

At 11:30 a.m. on July 13, 2021, Employee #1 was assembling a lift platform and inserting a torsion rod to hold the platform sections in place. The employee noticed the rod was broken, and as he removed the bolts securing it, the rod released a metal retainer that struck the employee in the face. Employee #1 was wearing safety glasses at the time. Emergency services transported the employee to the hospital, where he was hospitalized overnight for treatment of multiple facial bone fractures and a laceration.
