Employee is struck and killed by falling concrete pile
Investigation abstract
At 3:45 p.m. on October 6, 2021, an employee worked to cut a concrete pile with a power saw in a trench box. At some point, the twenty-eight-foot section of concrete fell onto and pinned the employee against the wall of the trench box. The employee was killed by chest injuries sustained.
