Event description

Employee sustains amputation of right pinky finger at sortin

Investigation abstract

At 9:30 a.m. on July 1, 2021, Employee #1, a temporary agency worker serving as citations were issued. lead packer at a produce handling facility, was working at sorting table #1 attached to a Giro machine in Room #5. On the first day of the season, while removing a moldy mandarin, the employee's right pinky finger was caught and amputated in the space between the roller and sheet metal at the end of the sorting table. A nearby coworker witnessed the incident, and the operations supervisor was present onsite in his office. The employee was hospitalized for treatment. Cal/OSHA investigated the incident starting July 20, 2021, and identified multiple causal factors, including unguarded rollers. Serious
