Employee amputates hand while operating unguarded shear
Investigation abstract
At 5:45 p.m. on June 4, 2021, Employee #1 was operating a Wysong 1010-PL shear to cut thin cardboard material. The hinged guard on the shear had been unknowingly propped up. While adjusting the cardboard material on the shear table, with his hands in the point of operation, Employee #1 inadvertently actuated the foot treadle. The shear blade descended and amputated the employee's right hand. The employee was hospitalized for treatment.
