Investigation abstract

At 9:18 a.m. on June 17, 2021, Employee #1 was placing debris in the back of a trailer truck. While standing inside the truck, he pulled a tarp over the debris when the tarp gave way, causing him to lose his footing and fall. Emergency services transported the employee to the hospital, where he was admitted and treated for fractured ribs.
