Employee fractures foot when struck by unbalanced load falli
Investigation abstract
At 7:30 a.m. on June 24, 2021, an employee was helping to move a 5,000-pound electrical cabinet using skates and jacks. As the load was being positioned, it became unbalanced and slid off one of the rear skates, landing on the employee's right foot. The employee sustained a fractured foot and was hospitalized. The employer had not provided safety training on the jacking and rolling method used to move the cabinet.
