Investigation abstract
At 8:00 a.m. on August 30, 2021, an employee who worked as a Health Assistant hospitalized on or about August 31, 2021. The employer had implemented required precautions to prevent the spread of COVID-19. and traveled to multiple locations for a school district was hospitalized with COVID-19. The school district had several cases of COVID-19 at the time of her hospitalization. Coworker #1, a Kindergarten Teacher, was hospitalized on or about August 31, 2021, and died from COVID-19 on September 18, 2021. Coworker #2, a Noon Duty Supervisor, was hospitalized on October 10, 2021. Coworker #3, a Health Assistant, was hospitalized on or about September 3, 2021.
