Event description

Employee falls from PIV elevated pallet, sustains concussion

Investigation abstract

At 2:07 p.m. on August 21, 2021, an employee who is the owner of a textile mill was working in the small warehouse. The employee requested that a coworker operating a Komatsu forklift lift him approximately 10 to 12 feet in order to access an overhead lighting system. The employee was attempting to adjust the position of one of the hanging lighting fixtures. The employee was standing on a wooden pallet resting on the forks of the forklift. After adjusting the lighting fixture, the coworker was lowering the platform when it shifted. The pallet fell along with the employee about 8 feet to the warehouse floor. The employee was hospitalized to treat head trauma to his forehead.
