Event description

Employee sustains partial amputation while performing pallet

Investigation abstract

At 2:42 p.m. on July 18, 2021, Employee #1, an industrial truck operator at a food distribution center, was performing a pallet replenishment in the cooler area. Replenishment involves moving a pallet of product from a reserve slot on a racking level above to a pick slot at ground level. After completing the replenishment, Employee #1 noticed an empty pallet in the second level of the racking partially sticking out. The exact sequence of events leading to the incident is unclear, and no witnesses were present. Employee #1 sustained a partial amputation of the left ring finger and was hospitalized for treatment.
