Employee with COVID-19 infection dies
Investigation abstract
On Monday, September 13, 2021, an employee had just returned to work as a tried to revive her but were unsuccessful. The OSHA 300 Log indicated that three coworkers had COVID-19 with time off between September 8, 2021, and September 19, 2021, including the employee. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative. Substance Abuse Counselor at a mental health and substance abuse facility after a getting over a stomach flu. On Friday, September 17, 2021, the employee started to experience COVID-19 related symptoms. On Saturday, September 18, 2021, the employee began to feel much worse and she quarantined herself at home. On Sunday, September 19, 2021, she took a COVID-19 test and tested positive. On September 26, 2021, the employee was hospitalized. At 4:00 a.m. on October 11, 2021, the employee died. There was a report that the employee's oxygen mask came off while in the hospital and was not immediately noticed. The hospital staff
