Employee partially amputates finger during carpet roller ope

Investigation abstract

At 8:30 a.m. on August 12, 2021, Employee #1, an Orr Operator, was operating Orr Carpet Roller Number 2 while being supervised at the carpet roller station. Employee #1, who was new to the station, was inserting a small carpet into the spindle for plastic wrapping. While adjusting the carpet with her right hand, the supervisor inadvertently actuated the spindle, leading to the partial amputation of Employee #1's right hand's middle finger. The employee was hospitalized for treatment.
