Event description
Employee fractures, lacerates foot in collision with PIV
Investigation abstract
At 8:30 p.m. on May 14, 2021, an employee working for a wood window and door manufacturer was operating a powered industrial vehicle (PIV) while moving doors to a staging area. The employee did not see a parked PIV located behind him. The employee struck the PIV at the forks where were elevated high enough to pierce the sole of his left shoe. The employee was pinned by his leg between the parked PIV forks and his own PIV. The employee was hospitalized to treat a fractured metatarsal and a lacerated foot.
