Event description

Employee amputates fingertip when reaching into riveting mac

Investigation abstract

At 9:00 a.m. on November 1, 2021, an employee was operating a riveting machine applying aluminum caps to the corners of cardboard material. The employee stuck his left hand underneath the guard to grab a cap that had popped off a corner and inadvertently pressed the foot pedal. The employee amputated his left index fingertip and required medical treatment wi thout hospitalization.
