Investigation abstract

At 8:37 a.m. on October 25, 2021, an employee was helping the rig team members with putting the rig swivel down when his finger was caught behind the swivel bell and was pinned between the swivel body and the bell. The employee was hospitalized to treat an amputation of his right middle fingertip.
