Investigation abstract
At 9:00 a.m. on June 11, 2021, Employee #1 was moving a stack of condensing units with a Clark forklift (model TMX15S) equipped with an overhead guard. The load was not balanced, braced, or secured while being lifted. When the stack shifted, the employee raised his hand to protect his head from a falling unit. His right middle and ring fingers were crushed between the unit and the forklift's overhead guard, resulting in amputations. The employee was hospitalized for treatment.
