Event description

Employee's fingertip is amputated after being caught by dock

Investigation abstract

At 9:30 p.m. on November 10, 2021, an employee was unable to find an extension tool to open a dock plate, so he used his hand to open it toward a parked tractor trailer. His right index finger got caught between the dock plate and trailer. The employee was hospitalized and had the tip of his right index finger surgically amputated.
