Event description

Employee with COVID-19 infection dies

Investigation abstract

An employee had transported a single patient who was later found to be COVID-19 was made aware of the exposure to a COVID-19 positive individual. The employee either had 1 dose of vaccine or was fully vaccinated at time of exposure. When he transported the positive individual, the employee wore a mask but was around the individual for longer than 15 minutes at a time. The employee began experiencing symptoms on November 4, 2021, and tested the same day. On November 11, 2021, the employee was admitted to a community hospital and on November 12, 2021, he died as a result of COVID-19. positive in his work van. The employer reported not knowing the patient was positive for COVID-19 at the time. The employee transported the patient on 4 to 5 different dates for dialysis appointments. The employee was believed to have been exposed either on October 27, 2021, or October 29, 2021. The employer has a policy to not transport COVID-19 positive patients and they found out through his medical records that the patient tested positive at a different facility when he was tested before receiving dialysis.
