Employee falls from elevation to lower level and incurs frac

Investigation abstract

At approximately 1:25 a.m. on November 24, 2021, an employee conducted a quality inspection of maintenance on a control module located on the roof of a light rail vehicle. The employee was conducting a quality assurance check of maintenance that was completed on the control module of light rail vehicle number 404. The vehicle was on track Y3 inside the maintenance shop. While conducting the check, the employee fell from the top of the rail vehicle, approximately 19 and a half feet, into the maintenance pit. The employee sustained fractures to his left leg and wrist and lacerations to his head. The employee was hospitalized for these injuries.
