Event description

Employee is struck by falling stack of cartons and sustains

Investigation abstract

At 8:30 a.m. on June 17, 2021, Employee #1, a maintenance janitor employed at a fresh salad production facility, was performing janitorial duties in a warehouse where packaging cartons were stored. The cartons were stacked approximately 12 feet high in three layers of pallets about 4 feet square and 4 feet tall each. While cleaning around the stacks, one pallet became unstable, causing a domino effect that resulted in a stack falling onto the employee. The employee sustained fractures to the left foot and left wrist, and a dislocated left shoulder. Emergency services transported the employee to Natividad Medical Center in Salinas, where he was admitted for treatment.
